The University of Notre Dame's response to the COVID-19 pandemic is designed to protect the safety of our students, faculty, staff and their families, continue our commitment to offering an unsurpassed undergraduate education that nurtures the mind, body and spirit, and enable us to seek to advance human understanding through scholarship, research and post-baccalaureate programs that heal, unify and enlighten.
To prepare for the upcoming spring 2021 semester, our office will contact the faculty and staff who previously requested an accommodation through our COVID-19 Accommodation Process. We will inquire to see if an extension of their accommodation is needed or reconsideration is desired for the spring semester.
The initial email contact to faculty and staff will start on Friday, October 2, 2020, and will continue on a rolling basis as needed.
Faculty and staff members who did not utilize the Accommodation Process in the fall, and whose age or health condition falls within one of the CDC High-Risk Categories or who have other special circumstances, may seek a workplace adjustment for the spring 2021 semester through the COVID-19 Accommodation Process by utilizing this form.

The Office of Institutional Equity will only accept accommodation requests submitted through the electronic form. It will not accept forms that are handwritten or physically mailed.
